• Shift: Day shift with on-call requirements (1-2 nights/week and 1-2 weekends/month)
• Shift Duration: 12-hour rotating shifts
• Estimated Weekly Salary: $1,680 - $1,770 per week

• Active Registered Nurse (RN) license required
• Minimum 1 year of nursing experience; home health or hospice experience preferred
• Current nursing certifications as applicable (BLS required; other certifications a plus)
• Colorado Adult Protective Services (CAPS) check required
• Reliable transportation required — limited public transportation in rural Gunnison
• Ability to participate in on-call rotation and provide travel nurse flexibility
• Strong assessment, clinical documentation, and patient education skills for homecare and palliative care

• Provide skilled nursing care, case management, and coordinated care for home health, hospice, and palliative patients
• Conduct initial home visits, comprehensive assessments, and ongoing patient reassessments
• Develop, implement, and update individualized plans of care in collaboration with interdisciplinary team members
• Administer nursing procedures, medications, and hospice interventions per scope of practice
• Complete accurate clinical and progress notes for home health documentation and billing
• Coordinate care with physicians, social workers, therapists, and community resources
• Provide patient and family education, counseling, and support for end-of-life and chronic illness care
• Supervise and mentor other nursing personnel and participate in ongoing professional development
