Location: Cripple Creek

The MDS Coordinator is responsible for the accurate and timely completion of the Minimum Data Set process to ensure regulatory compliance and appropriate clinical reimbursement. This role partners with nursing leadership and interdisciplinary team members to support documentation integrity, care plan accuracy, and case mix performance. The MDS Coordinator plays a critical role in maintaining survey readiness and supporting the financial health of the Skilled Nursing community.

Your Role:

  • Complete, review, and submit MDS assessments in accordance with federal and state regulations
  • Ensure PDPM accuracy and appropriate case mix index performance
  • Monitor clinical documentation to support accurate coding and reimbursement
  • Coordinate and participate in interdisciplinary care plan meetings
  • Track assessment schedules and ensure timely submissions
  • Collaborate with nursing leadership to identify documentation improvement opportunities
  • Support regulatory survey preparedness related to MDS and reimbursement
  • Educate clinical staff on documentation standards impacting reimbursement and compliance

Qualifications:

  • Active, unencumbered Registered Nurse license in CO
  • Minimum two years of clinical experience in Skilled Nursing or Long Term Care
  • Working knowledge of MDS 3.0 and PDPM reimbursement methodology
  • Strong understanding of Medicare and Medicaid regulations
  • Prior MDS coordination experience preferred
  • Strong analytical skills and attention to documentation detail
  • Ability to collaborate effectively with interdisciplinary teams
