In this role, you’ll translate clinical knowledge into compassionate, practical actions every day. Core responsibilities include administering prescribed medications and treatments under physician and RN supervision, monitoring residents’ vital signs, responding promptly to changes in condition, and coordinating with the interdisciplinary team to adapt care plans. You’ll assist with activities of daily living, provide wound care and preventive skin, fall-prevention, and safety measures, and deliver essential education to residents and families on disease management, meds safety, nutrition, and self-care strategies.

Documentation will be precise and timely to ensure seamless transitions between shifts and continuity of care.
