Join our team as a PRN Nurse Practitioner at Senior Comm Unity Care of Colorado PACE, Eckert. This PRN role provides primary care to participants in a PACE program serving older adults in Montrose and Delta Counties.
Pay: $57 – $67 per hour, based on experience.
Schedule: Clinic hours 8 am‑5 pm, with required evening and weekend on‑call. Occasional weekends are expected.

The Nurse Practitioner performs physical assessments of new PACE enrollees, semi‑annual reassessments of participants, and develops and implements appropriate plans of care. The role evaluates physical complaints, provides treatment, and delivers participant and caregiver education. The practitioner functions as a member of the Interdisciplinary Team (IDT).
Qualifications- Graduate from a professional nursing school.
- Licensed as Registered Nurse (R.N.) in Colorado.
- Certified as a nurse practitioner by a recognized national certifying organization.
- Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred.
- Minimum one year’s experience working with frail and elderly populations.
- Two years’ experience as a nurse practitioner in a geriatric setting desirable.
- Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
- Valid driver’s license, proof of insurance, and means of transportation.
- Skills and Knowledge:
- Knowledge of home and community‑based care for the elderly and disabled adults.
- Working knowledge of PACE regulations.
- Understanding of physical, mental, and social needs of frail older adults.
- Effective oral and written communication skills.
- Leadership and teamwork within an interdisciplinary setting.
- Strong organizational skills and ability to manage changing priorities.
- Proficiency with computers and electronic documentation tools.
- Conduct comprehensive in‑person history and physical assessments of new PACE participants.
- Collaborate with the IDT to develop comprehensive care plans.
- Perform semi‑annual reassessments and as needed follow‑up assessments.
- Address emergent and acute medical and non‑medical needs of participants.
- Participate in discharge planning for acute, skilled, and long‑term care settings.
- Manage chronic diseases and provide timely interventions in the clinic or participants’ homes.
- Refer participants to medical specialists as indicated.
- Oversee and provide medical care for PACE participants residing in Skilled Nursing Facilities (SNF).
- Participate in the on‑call rotation.
- Maintain complete medical records and fulfill charting and reporting requirements.
- Attend IDT meetings, communicate participant changes, and coordinate care decisions.
- Provide education and training to participants, caregivers, and staff.
- Assist with quality improvement initiatives and staff training.
- Perform other duties as required.
