Compensation- $40.00 – $48.00 per hour
- Sign-On Bonus: $5,000
- On-call compensation structure (availability pay plus hourly rate for visits)
- Graduate of an accredited school of nursing.
- CPR certification upon hire.
- Valid driver's license with reliable, insured vehicle for community travel.
- Strong assessment, communication, and clinical documentation skills.
- Ability to work independently and manage patient caseload effectively.
- Position serves both home health and hospice patients with flexibility between service lines.
- Travel throughout mountain communities required, especially challenging in winter conditions.
- On-call rotation shared among nursing team.
- Preferred:
Bachelor's Degree in Nursing (BSN). - 1-2 years of recent acute care experience in institutional setting.
- 1-2 years of recent home health or hospice experience.
- Experience with both home health and hospice service lines.
- Familiarity with rural healthcare delivery and mountain community needs.
- Provide comprehensive nursing care to patients and families in their homes across home health and hospice service lines.
- Conduct initial and ongoing assessments to determine patient and family care needs.
- Develop, implement, and regularly evaluate individualized care plans based on patient status and goals.
- Deliver skilled nursing interventions including medication administration, wound care, and treatments per physician orders.
- Serve as case manager for assigned caseload, coordinating care with interdisciplinary team members.
- Educate patients and families on self-care techniques, medication management, and health maintenance.
- Collaborate with physicians, therapists, and community resources to optimize patient outcomes.
- Maintain accurate clinical documentation and communicate changes in patient condition to care team.
- Support patients and families emotionally throughout care continuum, particularly in end-of-life care.
- Participate in on-call rotation providing after-hours patient support (reimbursed at Colorado minimum wage for availability, hourly rate for patient visits).
