This position will work with the Director of Juvenile Services to help manage and implement the House Bill 14‑51 Collaborative Management Project, named Family Advocacy and Support Team (FAST) and Statewide System of Care Implementation in Gunnison County. To assure that the mission, intent, and purpose of FAST is consistent with the HB 1451, the related Memorandum of Understanding and System of Care strategies, values and principles.
The Case Manager will work closely and collaborate directly with the school district to help meet youth and family needs. The Case Manager will sit on multiple multidisciplinary team’s (MDT) such as FAST, Gunnison County Substance Abuse Prevention Program (GCSAPP), Early Childhood Council (ECC), Child Welfare meetings (as appropriate) and any other teams deemed necessary and appropriate. The Case Manager is responsible for providing high‑level student support intervention, coordination of services to youth/families experiencing legal, academic/school, personal, or medical/mental health difficulties to assist them in removing barriers to success through goal‑oriented and strength‑based assessment to enhance their holistic well‑being of families.
The Case Manager will work with ISST’s and higher‑level case management. The case manager will facilitate team meetings, develop individualized case‑management plans, coordinates implementation of plans and monitors progress. The Case Manager works in partnership with all agencies involved to ensure youth and family needs are identified and a plan is put in place to achieve outcomes.
This position is currently supported by grant funding, which allows us to advance the county’s strategic initiatives. Employment beyond the current grant period is dependent on funding availability.
Pay RateThe hourly range is from 29.51 to 33.59, depending on experience, plus partial benefits.

Requirements- Must possess a valid Colorado driver’s license.
- Must complete IC 100 and IC 200 within the first year of employment.
- Bi‑lingual (Spanish) is required.
- Bachelor’s degree in human service/social service area.
- Or Experience in clinical social work, counseling or case management support and participation in community collaborations required.
