Business hours Monday - Friday with additional evening, weekend and holiday on-call expectations based off agency on-call rotation schedule.

The Residential team enriches the daily life of adults with IDD. Our staff develop and oversee a variety of residential settings and provide support and training to assist adults with IDD in practicing life skills and building confidence. FRIENDS believes in empowering Host Home Providers, Family Care Givers and direct care staff so that they can provide customized support to the individuals in their care.

Duties/Responsibilities

The Residential Coordinator is responsible for a variety of duties related to the overall wellbeing and independence of adults with a wide range of abilities. The staff also help ensure that people are living in safe and respectful homes and get the support they need to have a healthy and active lifestyle.

Duties include:

Position Duties and Responsibilities
  • Oversee the coordination and management of day-to-day residential program needs for an assigned caseload of individuals and supervision of Host Home and Family Caregiver, and independent living services.
  • Recruit, train and monitor Host Home Providers and Family Caregivers and ensure contract compliance.
  • Programmatic coordination and monitoring in accordance with individual's service plan.
  • Oversee all aspects of ensuring person centered supports for all adults enrolled in the Residential program and help them experience a high quality of life based on their abilities and wants.
  • Create detailed individualized plans, tracking, and monitoring of the person's progress.
  • Ensure agency and state licensing standards are adhered to in all Residential settings.
  • Monitor homes for safety and quality services.
  • Work regularly scheduled on-call shifts as part of agency staff rotation.
  • Additional duties as assigned by the Residential Director and/or Chief Operating Officer.
  • Meets weekly or as necessary with Residential Director and residential team.
  • Communicates with families, care providers, case managers, therapists and other professionals on participant matters.
Supervision
  • Oversee training and regular monitoring of all providers, including both onboarding and ongoing training and development.
  • Meet with residential providers on a monthly basis to provide support, training, and oversight in addition to home monitoring and quality assurance.
  • Support residential staff in implementing strategies for engaging participants and achieving highest level of independence as related to IP goals.
Administrative Duties
  • Maintain individuals and provider file tracking to ensure all documentation, plans, financials, and appointments are upheld in a timely manner.
  • Review individuals' information and ensure current information in files and Therap is accurate.
Other Duties/Responsibilities
  • Know, understand and implement agency policies and procedures, including but not limited to, protocols, safety guidelines, emergency procedures, communication processes, work schedules, vacation schedules, and professional standards.
  • Promote the Mission/Vision of FRIENDS, and contribute to on-going development of FRIENDS of Broomfield, Inc. and activities through suggestions to improve existing operational procedures.
  • Act as an advocate for the individuals with intellectual and developmental disabilities within the community, fostering and supporting a two-way relationship of respect and dignity between members and other community members.
  • Provide direct-care services, as needed.
Job Qualifications
  • Ability to perform basic computer operations - Microsoft Office Suite, Publisher, Excel worksheets, Internet, email, and Therap.
  • Pass a criminal background and CAPS check.
  • Possess a valid Colorado driver's license and ability to meet reporting agency's driving requirements.
  • Good written and spoken communication skills.
  • Ability to maintain detailed documentation, records, and reports.
