Conifer, Colorado (remote or hybrid position)
Schedule:
Part-Time, up to 30 hours per week
Starting Pay: $24.00 per hour

Position Summary:
We are seeking a motivated and compassionate Bachelor’s-level Case Manager t to join our care coordination team. This position provides direct support to clients through intake screenings, behavioral health assessments, individualized care coordination, and ongoing case management. The Case Manager plays a vital role in connecting members to mental health, substance use, medical, and social service resources to ensure continuity of care and improved outcomes.
- Conduct intake screenings and biopsychosocial assessments for new and existing clients
- Develop and update individualized care plans based on client needs and goals
- Provide care coordination among behavioral health providers, medical professionals, and community resources
- Monitor service delivery and support client engagement in treatment
- Maintain accurate and timely clinical documentation in compliance with Medicaid and agency standards
- Participate in team meetings, supervision, and ongoing training opportunities
- Advocate for clients and assist with barriers to accessing care
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (required)
- Experience in behavioral health, case management, or Medicaid programs preferred
- Strong understanding of community resources and social service systems
- Excellent communication, organizational, and documentation skills
- Ability to work independently while contributing to a collaborative team environment
- Must pass background check and meet all Colorado Medicaid provider requirements
- Starting pay: $24.00 per hour
- Part‑time schedule, up to 30 hours per week
