- Work with providers and clinical staff to obtain complete medical records for established patients using the medical records work list/work queue in the electronic health record (EHR).
- Document progress in the EHR for all tasks, including retrieval of pertinent patient materials; monitor and follow up on patient record statuses with standard processes and protocols.
- Capture paper documentation, prepare documents for scanning, scan and index documents within 1 hour of receipt; ensure 100% accuracy in assigning documents to the correct patient record and documentation type; perform QA on scanning and address errors with clinical staff; train staff on scanning and indexing.
- Respond to requests for copies of patient information from providers, clinical staff, patients, insurers, attorneys, and state/federal agencies; understand release of information policies and abide by HIPAA and applicable laws; log releases and prepare copies in paper, CD, PDF, fax, or other approved formats; accommodate same-day requests when possible.
- Assist with processing paperwork and administrative tasks related to clinical care, including managing records requests, orders, scanning, and uploading records.
- Resolve problems independently, maintain continuous communication with clinical/non-clinical team members, and escalate issues to leadership when needed.
- Recognize emergencies and respond using standard operating procedures and critical thinking.
- Cross-train as part of the business office team and cover roles as needed (e.g., Patient Access Representative I).
- Coordinate with other business office team members when out of the office to maintain HIM activity continuity.

- Performs other duties as assigned and maintain HIPAA compliance.
- One year of medical office or clinical experience required.
- Working knowledge of medical terminology preferred.
- Prior experience with medical records; collecting, scanning, and requesting preferred.
- Bachelor's degree in related field preferred.

- Use of computer, keyboard, and mouse with basic Microsoft Office applications required.
- Typing skills of no less than 20 WPM required.
- Ability to search resources/Internet to locate physician and healthcare facility information to make appropriate decisions.
- Computer skills necessary to complete work assignments, online learning requirements, access online forms and policies, and complete online benefits enrollment.
- Must have working knowledge of English (reading, writing, speaking).
Pay is based upon relevant education and experience per hour. Hourly pay: $21.32 - $25.40 USD
.
