Key Rehab is looking for a Physical Therapist in either a Full‑time, Part‑time, or PRN capacity to join our team in providing excellent care in the
Burlington, CO area in a SNF setting.

• Evaluate each patient, create treatment plans, and collaborate with the interdisciplinary team to help patients reach their goals.
• Supervise PTAs within the facilities.
• Work in both Home Health and SNF settings, offering a weekend schedule that may reduce the need for childcare at home.

• Accredited education:
  Graduation from an accredited Physical Therapist program.
• Active licensure:
  Current PT license in the state of practice.li>
• Ethical compliance:
  Adherence to Key Rehab’s Code of Ethical Conduct and state/federal regulations.

• Strong organizational and problem‑solving skills.
• Flexibility and team collaboration.
• Exceptional communication skills.
• Experience with geriatric population.
• Knowledge of Medicare billing regulations.
