LICENSES & CREDENTIALING (REQUIRED)

Active Arizona medical license

or

IMLC eligible for Compact license

Clinically active within the past year

CERTIFICATIONS & EXPERIENCE (REQUIRED)

Board Certified in Neonatology

Minimum 1 year post-fellowship experience

Minimum Level III NICU experience

At least 6 months clinical experience in neonatology role

Ability to offer ongoing availability
