Colorado Registered Nurse

Overview

The Registered Nurse (RN) provides skilled nursing care to patients on an intermittent basis in their place of residence. The RN may also provide private duty nursing services outside the home to assist patients with activities of daily living when needed. Care is delivered in accordance with physician or authorized practitioner orders and the patient's plan of care, under the supervision of the Administrator and Clinical Manager.

• Provide skilled nursing care according to physician or authorized practitioner orders and established care plans.
• Conduct initial patient assessments and ongoing evaluations to determine nursing needs.
• Develop, implement, and revise individualized plans of care.
• Deliver specialized nursing services, including preventive and rehabilitative care.
• Monitor patient conditions and promptly report changes to physicians and appropriate team members.
• Document patient visits, progress notes, and care conference summaries in a timely and accurate manner.
• Educate patients and families on care needs, treatment plans, and disease management.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Participate in patient care conferences, staff meetings, and quality improvement initiatives.
• Complete required clinical documentation including OASIS assessments, recertifications, transfers, and discharge summaries within agency time frames.
• Supervise and support LPNs and other nursing personnel as assigned.
• Participate in on-call rotation for evenings, weekends, and holidays as required.
• Maintain patient confidentiality and adhere to HIPAA regulations and agency policies.
• Perform additional duties as assigned.
