About the RoleWe are hiring a passionate, motivated provider to join us full‑time and independently offer care and consultations at our contracted facilities.
Day‑to‑Day Responsibilities- Round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers.
- Deliver specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
- Perform focused assessments and develop individualized treatment plans in collaboration with facility primary care teams.
- Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs.
- Serve as the sole physiatrist on‑site, operating autonomously while supported by a collaborative clinical team for consultation as needed.
- Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry.
- Current, unrestricted PA or NP license.
- DEA licensed or willing to obtain DEA licensure.
- Experience in nursing homes and/or physiatry is a plus, but not required.
- Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
- Strong communication skills and ability to collaborate with multidisciplinary teams.
- Familiarity with EHR charting.
- Genuine passion for providing professional, personalized care in an autonomous environment.
- Hands‑on training in facility settings to ensure consistent, high‑quality care.
- Flexible 5‑day workweek with flexible hours; no nights or on‑call shifts.
