Physiatry APP (Physician Assistant / Nurse Practitioner)

Iconic Care is a physician‑owned group of skilled providers (physicians, PAs, and NPs) offering care across ten states, with plans for expansion to all 50 states. We partner with top‑tier nursing and skilled care facilities nationwide to manage pain and enhance therapy outcomes.

We are hiring a passionate, motivated provider to join us full‑time and independently offer care and consultations at our contracted facilities.

• Round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers.
• Deliver specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.
• Perform focused assessments and develop individualized treatment plans in collaboration with facility primary care teams.
• Provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient‑specific needs.
• Serve as the sole physiatrist on‑site, operating autonomously while supported by a collaborative clinical team for consultation as needed.
• Coordinate care and refer to other specialties or services for issues beyond the scope of physiatry.

• Current, unrestricted PA or NP license.
• DEA licensed or willing to obtain DEA licensure.
• Experience in nursing homes and/or physiatry is a plus, but not required.
• Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
• Strong communication skills and ability to collaborate with multidisciplinary teams.
• Familiarity with EHR charting.
• Genuine passion for providing professional, personalized care in an autonomous environment.
