ProfileAs an on‑call RN/LPN at Bloom hospice, your primary responsibility is to provide and coordinate hospice care for patients and families during after‑hours. You collaborate with the interdisciplinary hospice team to ensure patients' physical, emotional, and spiritual needs are met. You educate patients and families about disease processes, manage and control symptoms, and offer support during difficult times. Care delivery primarily occurs in patients' homes or community settings.
Responsibilities- Provide hospice patient care after‑hours as needs arise
- Conduct effective pain/symptom assessment and management
- Deliver comprehensive patient/family/caregiver teaching
- Manage end‑of‑life ethical issues effectively
- Create timely and accurate documentation
- Utilize resources effectively and efficiently
- Adhere to scope of practice and continuously improve processes and services
- Maintain and enhance professional skills
- Adhere to high standards of personal and professional conduct
- Graduate of an accredited nursing school
- Current Colorado RN/LPN licensure in good standing
- Minimum one year of recent medical, surgical, or critical care nursing experience
- Strong clinical assessment skills
- Excellent communication and interpersonal skills
- Solution‑driven, creative, and resourceful problem‑solving skills
- Proficient computer skills
- Previous hospice or oncology experience
- Knowledge of death/dying and bereavement processes
- CHPN certification

The salary for this position is competitive and commensurate with experience. The pay range for this role in Colorado typically falls between $75,000 and $100,000 annually, with potential for performance‑based bonuses and other benefits.
