Job Posting: RN Case Manager - Colorado Springs/ Pueblo. Pivot Home Health is a locally owned, employee-first home health agency serving Colorado communities. We’re looking for a compassionate and motivated Registered Nurse (RN) Case Manager to join our growing team. As an RN Case Manager, you’ll provide direct patient care, oversee individualized care plans, and support families through their home health journey.

• Salary Range: $75,000 – $85,000 annually (DOE)

• Manage and coordinate the care of patients in their homes
• Perform initial assessments, recertifications, develop care plans, and provide skilled nursing care
• Collaborate with physicians, therapists, other community partners, and caregivers to ensure optimal outcomes
• Educate patients and families on managing conditions and medications
• Document accurately and timely in the EMR
• Participate in case conferences and team meetings
• SN visits as needed

• Current RN license in Colorado (BSN preferred)
• Minimum 1 year of nursing experience (home health or acute care preferred)
• Strong assessment, clinical, and communication skills
• Ability to work independently while being part of a team
