Job Overview
The Registered Nurse (RN) Case Manager develops, coordinates, implements, and evaluates individualized care plans. This role works in collaboration with patients, families, healthcare team members, community providers, and payers to ensure appropriate level of care, placement, length of stay, follow‑up care, outcomes, and reimbursement.

Job Status: part time, 44 hours per pay period, eligible for benefits
Shift: per pay period, three weekdays (each shift 8‑hour) plus every other weekend Saturday and Sunday (10‑hour shifts)
Relocation assistance available for relocations from 100 miles or greater.
Duties & Responsibilities- Assesses, plans, facilitates, and advocates for optimal clinical and financial outcomes.
- Participates in department initiatives, projects, and committees as assigned.
- Gathers and evaluates information including medical needs, payer coverage, family dynamics, and community resources to create individualized care plans.
- Collaborates with families in developing and updating care plans.
- Coordinates a range of case‑management functions such as admissions, transfers, utilization review, discharge planning, transportation, and ongoing care coordination.
- Communicates and partners with internal teams and external providers to support continuity of care.
- Advocates for the effective and appropriate use of available resources.
- Documents interventions and activities aligned with patient, family, and organizational goals.
- Adjusts care strategies and processes in response to changes in patient condition or circumstances.
- Uses data to inform decisions, improve outcomes, and ensure efficient resource use.
- Education:
Bachelor of Science in Nursing (BSN) - Experience:
Three years of nursing experience in a pediatric setting or three years of case‑management experience - Licensure:
Registered Nurse License
Hourly Range: $40.26 to $60.39
