Human Touch Home Care is seeking a compassionate and dedicated Home Healthcare License Practical Nurse (LPN). Our Home Health LPNs deliver high-quality nursing care to patients in their homes under the supervision of a registered nurse (RN). This role involves assessing patient needs, implementing care plans, and assisting with daily activities to enhance patient well-being.

Sign-on Bonus: $5,000

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, reporting identified needs to the registered nurse and/or clinical supervisor.
• Uses and/or prepares equipment and supplies effectively and adheres to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

• Graduate of an accredited practical nurse or vocational nursing program
• Three (3) years of nursing experience; community/home health or medical/surgical experience preferred
• Currently licensed as an LPN/LVN in the state
• Valid CPR certification
• Complies with accepted professional standards and practice
• Good verbal and written communication and organizational skills

• Full-time, Part-time

Monday to Friday; weekends as needed

1 year (Preferred)
