We are seeking a dynamic, compassionate Brookdale at Home Manager to lead our in‑home care program and ensure clients receive exceptional service right where they are most comfortable.
As a strong leader who thrives at the intersection of operations, people, and purpose, you will manage the Home Care Agency, oversee staffing, quality, clinical programs, and budgeting. You will promote a positive workplace culture and drive continuous improvement.
Key Responsibilities- Manage the Brookdale at Home agency, ensuring efficient and effective operations for 10‑20 associates.
- Motivate associates through recognition, incentives, and grievance resolution.
- Provide ongoing orientation, training, and education to staff in classroom and in‑service settings.
- Ensure all policies are disseminated and followed by all associates.
- Contribute to the development and implementation of the annual budget.
- Collaborate with community associates, administrators, and clinical staff on agency services and clinical programs.
- Maintain an effective Quality Assurance Program.
- Partner with community administration in preparing for state survey and other regulatory processes.
- Market the agency to appropriate audiences.
- Coordinate clinical services for the agency, if applicable.
- Manage billing, scheduling, and delegation of services.
- Manage documentation to ensure completeness, accuracy, and timeliness.
- Comply with safety, infection control, universal precautions, and OSHA standards.
- Maintain current knowledge of national and state requirements for scope of practice, supervision, credentialing, and continuing education.
- Demonstrate sound judgment in evaluating, planning, implementing, and following up.
- Maintain positive relationships with coworkers, residents, family members, and community personnel.
- Participate in community committees such as Daily Stand‑Up, Collaborative Care, and Resident Council.
- Handle after‑hours and weekend on‑call duties.
- Respond to calls after hours and weekends in a timely manner.
- Cover BAH cases when care associates cannot be secured.
- Manage on‑call and after‑office hours business needs as assigned.
- Bachelor’s Degree preferred.
- 3‑5 years’ experience in healthcare.
- LPN or RN preferred; at least one year of supervisory experience required.
- Apply existing guidelines to make varied decisions within the department.
- Possess extensive knowledge of a distinct skill or function and thorough understanding of the organization.
- Effective oral and written communication skills; strong supervisory, organizational, and problem‑solving abilities.
- High degree of accuracy, initiative, and creativity; ability to operate independently and manage time efficiently.
- Professional ethics and a positive, professional image at all times.
- Ability to manage multiple priorities and tasks.
- Standing, walking, sitting, using hands and fingers.
- Reach with hands and arms; stoop, kneel, crouch, or crawl.
- Talk or hear; lift up to 50 pounds.
- Possible exposure to communicable diseases; exposure to latex, blood‑borne pathogens, and other chemical hazards.
- Occasional weekend, evening, or night work; on‑call as needed.
- Requirement for occasional travel and driving (Tier
2).
