Job Title: QMAP/Caregiver
Status: PRN

Job Summary

The Med Care Manager/QMAP must administer, pour, pass, and document all routinely prescribed medications per physician orders under the direction of the Wellness Director and as approved by the State Medication Administration Regulations. You will learn each resident’s medication preferences. Some residents may engage playfully as you administer medications.

Position Overview
  • Administer and document regularly prescribed medications per the Medication Administration Record, observing the five rights for safe medication administration and per physician orders.
  • Order medications from the pharmacy as prescribed, 7 days prior to running out, and as directed by the Wellness Director to ensure availability; notify if not arrived within 24 hours.
  • Immediately after administering medication, document in the resident's medical record; date and initial the bubble pack after removing from it.
  • Observe and report reactions and side effects of medications to the licensed nurse.
  • Take and record vital signs prior to administration when required or ordered.
  • Administer PRN medications per physician order.
  • Report any medication errors to the Wellness Director immediately.
  • Keep the medication cart clean, stocked, and locked with the medication administration book inside; carry the cart key with you at all times during your shift.
  • Assist with development and review of the resident's care plan in conjunction with other disciplines.
  • Ensure residents’ privacy, respect, and dignity.
  • Detect, correct, and report unsafe conditions which may harm a resident.
  • Support physical comfort, safety, and mental well-being of residents.
  • Assist in daily tasks such as toileting, grooming, bathing, feeding, dressing, monitoring intake, hair/nail care, moving between areas, and responding to calls.
Job Qualifications

  • License/Certification:
    State-approved Medication Administration training;
    First Aid as required by ALF regulations; QMAP in Colorado.
  • Education/

    Experience:

    Minimum high school diploma or equivalent. Prefer experience in health care, especially in Assisted Living or Memory Care; experience administering medication in an assisted living setting. Ability to make decisions with residents’ interests in mind and in line with company policies.
  • Continuing

    Education:

    As required by law; monthly in-service education within the community.
  • Other:
    Proficient in reading and writing English. Ability to read and interpret safety and operation documents; good communication skills; ability to understand and follow written and oral directions. Provide maximum resident-care services within delegated tasks; exercise good judgement in recognizing changes in condition. Work with licensed nurses and know your limitations.
  • Job Knowledge:
    Knowledge of Medication Administration Procedures and universal precautions to prevent infection.
  • Industry

    Experience:

    Minimum three years in senior living preferred; passion for working with seniors; experience in an Assisted Living community.
  • Team members scheduled for 24+ hours per week are eligible for benefits; families of team members may also be eligible.
