• Provide patient-centered care under the supervision of a Registered Nurse
• Complete proper documentation using Home Care Home Base EMR
• Assist clients with personal care or home management tasks, including basic hygiene, feeding, ambulation, medical monitoring, and basic health‑care related tasks
• Assist basic household tasks such as housekeeping, shopping, and other related tasks

• Minimum age 18 years
• Valid driver’s license with satisfactory driving record as defined by agency policy
• Certification required by state licensure as a CNA or Home Health Care Aide in the state in which the aide provides care
• Must be able to use proper body mechanics while lifting (may be expected to lift up to 75 lbs.) and be able to sit, stand, walk, bend, stoop, push, pull, and reach for periods of time
