This position collaborates with the integrated clinical care team to implement services for the Healthcare for the Homeless program, which involves community outreach and individual care coordination. Utilizes a variety of outreach approaches and care coordination models to work with a diverse client population; works directly with the Engagement Services Manager to deliver a consistent outreach message.

Salary: Starting at $20.00 (Starting $17.50 + locale differential $2.50 = $20.00) - $21.05 per hour (non-exempt - hourly) and considers preferred experience and education described above. This position includes a $2.50 locale differential at our Durango worksites.

Physical demands: Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between Axis Health System locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate.

Required Skills
- Knowledge of mental illness, substance abuse, and psychopathology and associated treatment modalities.
- A working knowledge of individual, group, family and crisis treatment approaches and their application to patients with emotional and behavioral problems.
- Working knowledge of HIPAA and 42 CFR, and ability to regard all work as confidential.
- Knowledge of community health and mental health resources and their eligibility criteria.
- Ability to work independently with minimal supervision.
- Working knowledge of Windows based computer applications to include MS Word, MS Excel, MS Outlook, and Internet Access.
- Ability to operate standard office equipment including calculator, copy machine, fax and multi-line telephone. Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
- Knowledge of or ability to learn new patient documentation systems.
- Ability to organize and prioritize workload in a sometimes hectic environment with frequent interruptions.
- Must have current driver's license and ability to be commercially insured and maintain a driving record that allows insurability with AHS commercial insurance agency.
- Ability to take on additional duties as necessary.
Additional eligibility requirements: Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.
