• Provider is responsible for rounding and managing the clinical care of the population at the assigned SNF.
• Provider prescribes medications and treatments for residents residing at SNFs.
• Provider delivers high‑quality, patient‑centered, comprehensive primary care services to patients in SNFs.
• Provider documents all patient care/encounters in a timely manner.
• Provider is available for phone/secure‑messaging consultations with facility nursing staff.
• Provider records are completed in Theoria’s proprietary electronic health record (EHR) system, Chart Easy™.
• Commonly used facility EHR systems include Point Click Care , Matrix Care, and American Health Tech.
• Provider is expected to be on‑site on time for scheduled shifts.
• Timely attendance and reliable transportation are required.

Rounding time is 7:00 AM–1:00 PM local time, during which the provider must be available to nursing staff and address acute/emergent issues that arise. Shifts begin around 7:00 AM local time with flexibility within reason. Shifts are daily Monday‑Friday. Charting/ documentation can be completed off‑site. This is an in‑person position.

• Competitive pay up to $135,000 per year, based on position type (full‑time, part‑time, or PRN).
