H2 Health is seeking a Certified Occupational Therapist Assistant to join our Prime Living team. The therapist must be able to work independently and in teams and make appropriate decisions based on each patient s functional needs.

Qualifications:

• Graduate of an accredited Certified Occupational Therapy Assistant program.
• Active or eligible for state license.
• Prior experience in an outpatient setting is preferred, but not required.

Responsibilities:

• Provide Occupational Therapy services as a Certified Occupational Therapy Assistant in outpatient settings.
• Work independently and in collaboration with a multidisciplinary team to meet patient goals.
• Make appropriate clinical decisions based on each patient s functional needs.
