Home Health Patient Care Manager​/Registered Nurse

We are seeking a dedicated and experienced Patient Care Manager / Registered Nurse specializing in Home Health to join our team in College Station. This role requires a compassionate and skilled professional committed to delivering high-quality care to our patients in their homes.

• Oversee and manage patient care plans, ensuring they are effectively implemented and updated as necessary.
• Lead and support a team of healthcare professionals, fostering a collaborative and positive environment.
• Conduct regular assessments of patient needs and adjust care plans accordingly.
• Maintain accurate and up-to-date patient records in compliance with health regulations and standards.
• Coordinate with other healthcare providers to ensure comprehensive and continuous patient care.
• Provide direct patient care as required, maintaining a high standard of clinical excellence.

• Registered Nurse (RN) with a valid license to practice in Texas.
• Minimum of 3 years of nursing experience, with at least 1 year in a home health setting.
• Proven leadership and management skills, with the ability to inspire and motivate teams.
• Strong organizational skills and attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and make sound clinical decisions.
