The Nurse Navigator is a Registered Nurse who facilitates patient continuity of care with the healthcare team in an efficient and timely manner. The RN acts in an intake model, assessing patient needs, assisting with planning, organizing, implementing, and evaluating patient care with the healthcare team. The Nurse Navigator functions autonomously and in collaboration with other members of the healthcare team as a patient advocate, educator, and coordinator.

The Nurse Navigator possesses strong verbal and written communication skills.

Essential Functions Of

The Role
  • Utilize the nursing process of assessment, planning, implementation, and evaluation of patient care according to physician orders and establish a plan of care.
  • Coordinate with physician’s office to schedule and perform necessary testing for definitive diagnosis.
  • Communicate with all members of the healthcare team involved in providing patient care.
  • Facilitate timely access to quality medical care from diagnosis through survivorship.
  • Collaborate with physicians and other health care providers to improve patient care within an interdisciplinary model.
  • Educate patients and family members on diagnosis, treatment options, and available services.
  • Provide direction and support to patients and families, serving as patient advocate.
  • Maintain accurate and complete nursing documentation in the Patient Navigator database.
  • Act as a point of contact for patients throughout the continuum of care.
  • Participate in case conferences to maintain and increase disease-specific knowledge.
  • Participate in continuous quality improvement projects to better serve patients, families, and the healthcare system.
  • Utilize leadership skills such as decision making and delegation to ensure safe patient care in a professional manner.
  • Serve as a liaison between patients and physician/office staff on a regular basis.
  • Provide facility information, directions, and general assistance (transportation, hotels, etc.).
  • Assist with patient escort throughout the hospital for hard-to-find locations.
  • Follow up with patients at 3-month and 1-year intervals.
  • Participate in community service events.
  • Assist with onboarding new nurse navigators to the program.

Qualifications
  • Education:

    Bachelor’s degree
  • Experience:

    3 years of experience
  • Certification/License/Registration:
    Registered Nurse (RN)
  • Basic Life Support (BLS): BLS within 30 days of hire or transfer
