** Lead the Way in Hospice and Palliative Care
** knowledgeable and passionate about hospice and palliative care to coordinate and drive business growth while fostering collaborative relationships with key healthcare providers and community members.

* Key Responsibilities:
**
* ** Referral Generation and Management:
** Proactively identify, develop, and maintain strong relationships with referral sources such as hospitals, skilled nursing facilities, and community organizations.
* ** Transition of Care Coordination:
** Ensure seamless and efficient transitions of care for patients, working closely with the clinical management team and other stakeholders.
* ** Industry Knowledge and
Education:
** Stay abreast of industry trends and best practices in hospice and palliative care. Educate healthcare professionals and community partners on our services.
* ** Strategic Partnerships:
** Build and nurture strategic partnerships with key stakeholders to enhance our market position.

* Qualifications:
**
* ** Essential:** + Current RN licensure or compact licensure recognized by the State of Vermont + Minimum 5 years of experience in the hospice field
* ** Preferred:** + Hospice & Palliative Care Certification (or willingness to obtain within 1 year) + Advanced training in Serious Illness Conversations (e.g., Talk Vermont) + Experience in sales, marketing, and liaison roles
