Team Assistant II - Specialty Care – UVM Health - Home Health & Hospice
Position SummaryThe team assistant is responsible for scheduling staff of specific disciplines and providing clerical and administrative support necessary to assure delivery of the best possible care to patients and families. Key responsibilities include receiving and triaging team phone calls, coordinating services, and communicating with patients, physicians and staff.
Minimum Qualification Standards Education- Associate’s degree in business or another appropriate discipline.
- Two to three years of clerical and administrative experience, or a combination of education and experience that provides comparable knowledge and skills.

- Must be able to speak, read and write in English.
- Honest, self‑directed, possessing initiative, good judgment and a positive attitude toward daily activities.
- Clear and effective written and oral communication skills.
- Ability to work cooperatively with employees, clients, customers and the general public.
- Maintain satisfactory standing with the Adult Abuse Registry, Child Abuse & Neglect Registry, Vermont Criminal Information Center and OIG.
- Be free from the illegal use and effects of drugs and alcohol in the workplace.
- Perform job functions without risk of harm to self or others.
- Valid driver’s license with reliable, registered, insured vehicle, as required by the organization and appropriate to the position.
- Previous experience in a medical setting with working knowledge of medical terminology and the ability to converse comfortably with patients, staff and external individuals.
- Travel may be required.
- May work in community‑based settings, clients’ homes, and outdoors.
- May work during non‑business hours.
- May encounter hostile or emotionally upset clients, family members, visitors and staff.
- Expect stress due to a demanding profession.
- Exposure to blood‑borne pathogens.
- Exposure to inclement weather, animals, firearms, tobacco use, dust and other client‑based activities and preferences.
Physical Requirements
- Walk, sit, stand, bend, lift and move intermittently during working hours.
- Push, pull and lift from the floor up to a total of 50 pounds.
- Navigate stairs of varying heights and degrees.
- Maintain balance.
- Perform kneeling, crouching, stooping, twisting, reaching below waist level, overhead and torso level.
- Manual finger dexterity required to use equipment and perform duties.
- Visual acuity sufficient for driving, using office equipment and performing duties.
- Auditory acuity to assist individuals in person and by telephone.
- Receive referrals from Intake, schedule admissions, and assign a case manager.
- Coordinate scheduling for a discipline team on an ongoing basis and track field staff schedules.
- Assign patients to appropriate interdisciplinary team members.
- Maintain effective communication with field staff and Clinical Team Manager.
- Respond to all telephone calls directed to the team and pass appropriate information to team members.
- Schedule meetings as requested and take minutes as needed.
- Track referrals and admission documentation within 24 hours and manage transfers.
- Support the team in meeting clinical, patient satisfaction and financial outcome goals.
- Type, proofread and send correspondence; create and maintain forms, reports, logos and other documents.
- Prepare lab requisitions as requested.
- Order and maintain inventory of office and clinical supplies and equipment.
- Establish and maintain on‑call schedules.
- Maintain calendar of time off approved by Clinical Team Manager.
- Assist with preparations for weekend work.
- Adhere to confidentiality policies and procedures.
- Provide exemplary customer service to all internal and external customers.
- Demonstrate accountability and follow‑through of all designated tasks.
- Maintain commitment to the IDT team and associated patients.
- Perform any other duties as assigned by supervisor.
