About the Role
🏥 We’re seeking an experienced Medical Social Worker to support Home Health and Hospice patients in the Colchester, VT area. This role focuses on providing compassionate, patient-centered social work services in the community.
What You’ll Do- Provide medical social work services for Home Health and Hospice patients
- Conduct psychosocial assessments and care planning
- Support patients and families with resources, counseling, and end-of-life planning
- Collaborate with interdisciplinary clinical teams
- Document care accurately in EMR systems
- Master’s Degree in Social Work (MSW) –
Required
- 2+ years of relevant Medical Social Work experience
- Epic EMR experience within the past 24 months – Required
- Current EMR:
Netsmart | Transitioning to Epic
- Reliable personal vehicle required
- Must be comfortable driving in winter/snow conditions
- Snow tires required
- Public transportation not accessible in this area
- Competitive weekly travel pay
- Short-term assignment with strong clinical impact
