** About the Role
** As an
** Adult Home Care Physical Therapist**, you will:
* Perform comprehensive evaluations and assessments
* Develop individualized treatment plans tailored to each patient’s needs
* Deliver skilled therapy interventions to improve mobility, strength, and function
* Support patients in remaining in their homes safely and independently Our coverage area is segmented into four territories within
** Chittenden and Grand Isle Counties**. Case load territory is assigned after orientation.###
** Minimum Qualifications*
* * Bachelor’s degree or higher in
** Physical Therapy
*** 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
* ** Home health experience preferred, but not required. We will mentor and support you.
*** Current Vermont Physical Therapist license, or willingness to obtain one ###
