As a home health physical therapist
, you'll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff , providing comprehensive, patient-centered care.

As an Adult Home Care Physical Therapist
, you will:
- Perform comprehensive evaluations and assessments
- Develop individualized treatment plans tailored to each patient's needs
- Deliver skilled therapy interventions to improve mobility, strength, and function
- Support patients in remaining in their homes safely and independently
Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties
. Case load territory is assigned after orientation.
- Bachelor's degree or higher in Physical Therapy
- 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
- Home health experience preferred, but not required. We will mentor and support you.
- Current Vermont Physical Therapist license, or willingness to obtain one
