We’re looking for a dedicated Social Worker to provide supportive counseling, care coordination, and psychosocial assessments for individuals facing illness and disability.##
** Education & Licensure
*** Master’s Degree in Social Work (MSW) required
* Vermont
** LMSW
* * required;
** LICSW
* * preferred* At least 2 years of relevant experience (healthcare experience preferred)##

Skills & Qualifications
*** Strong communication, counseling, and rapport-building skills
* Ability to assess psychosocial needs and explain medical information clearly
* Experience with crisis intervention, community resources, and care management
* Strong knowledge of local community resources
* Knowledge of Medicare/Medicaid, insurance processes, and mandated reporting
* Reliable transportation and valid driver’s license (travel required)
* Ability to work collaboratively with patients, families, and interdisciplinary teams##
** What You’ll Do
*** Complete comprehensive in-home psychosocial assessments
* Support patients and families in coping with illness and disability
* Coordinate care plans with medical team and community partners
* Provide crisis intervention, short-term counseling, and education
* Document assessments and care plans according to regulatory guidelines
* Participate in team meetings, care conferences, and quality improvement activities##
** Working Conditions
*** Work in patient homes, community settings, and varied environments
* Occasional exposure to emotional situations or environmental stressors
