Colchester. Working Style:
Community-based worker. Salary: £25,395 to £32,131 per annum.
We have an exciting opportunity within Early Intervention in North Essex for a Community Support Worker. You’ll be working within the Discharge to Assess Team 2, based in Colchester, covering all areas of North East Essex. Responsibilities- Undertake assessments and reviews in line with statutory duties under the Care Act to identify eligible needs and generate a person-centred, outcome-focused support plan.
- Work in partnership and develop relationships with internal and external colleagues/partners to achieve the best outcomes for Adults and Carers.
- Engage in a community-based role, visiting residents in their homes, meeting colleagues, and working from the office or from home as required.
- Be flexible to work a 6-week rolling rota including 1 weekend every 6 weeks and 1 bank holiday per year; 6% allowance provided as compensation.
- Travel across the operational area; driving licence and access to a vehicle (or suitable alternative) required.
- Attend the office in Colchester approximately 2-3 days per week in addition to business needs.
- Engage in induction and training; commitment to learning and development.
- Curious by nature; ability to think creatively to engage with the community and achieve outcomes.
- Strong ability to build effective relationships with adults and their families in times of challenge and crisis.
- Drive and passion to make a difference; willingness to learn.
- Ideally some experience of activity-based work with adults and knowledge of legislative and policy guidance applicable within social care settings.
- Training and development through the Essex Social Care Academy.
