Overview
The Integrated Discharge Team works with patients and families with complex needs and conditions, supporting them in the discharge process in order for a smooth transition into the community. We aim to anticipate, co-ordinate and join up health and social care needs for patients at a high risk of unplanned admissions to hospital. We provide guidance and professional support to the patient and the patient's families, and work with them in partnership to ensure the patient gets the most appropriate plan in place for discharge, while ensuring that interventions are not duplicated.
This process improves the quality of care as well as improving patient satisfaction. The person is at the centre of holistic care planning, working within Southend hospital alongside other organisations such as social care and the voluntary sector. Our aim is to promote and work towards an Integrated Discharge Team way of working as discussed within the NHS Long Term Plan.

The Integrated Discharge Team supports patients with complex healthcare needs, who may require long-term health and social care planning or a short-term intervention following a change of condition or acute illness.
- Assess patient needs and plan and facilitate safe and robust discharges, working alongside the hospital MDT and external system partners to ensure patients are discharged from the acute setting and aid in maintaining patient flow throughout the acute trust.
- Provide leadership support by managing junior members of staff with support of the Operational Lead to ensure effective leadership and good working relationships.

Band 7. Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population. Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust.
There are also three main community and mental health service providers who work together through a community collaborative. We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.
