JOB SUMMARY

The Home Health and Hospice RN provides skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span, possess the ability to assess data reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and provide the care needed as described in the unit's/area's/department's policies and procedures.

JOB REQUIREMENTS

Current Wyoming licensure as a Registered Nurse. Position requires travel around a 50-mile radius of Cody, reliable transportation necessary. Valid driver's license and vehicular insurance. Must be available for rotating work and call schedule.

EXPERIENCE

Home Care experience preferred. Two (2) years nursing experience required.

REQUIRED CERTIFICATIONS

BLS (AHA) and a current driver's license with clean driving record.

ESSENTIAL FUNCTIONS

• Nursing assessments and patient evaluations.
• Regularly re-evaluates the nursing needs of patients and prepares re-certification if appropriate.
• Initiate the patient's plan of treatment and any necessary revisions.
• Routinely perform 4-5 nursing visits/day and more if indicated.
• Inform MD and Clinical Supervisor of changes in patients' condition and needs.
• Comply with Medicare Conditions of Participation.
• Provide complete patient care and Case Management for a team of patients.
• Document all cares and notes in Home Care system.
• Perform supervisory visits of Home Health and Hospice Aide.
• Provide appropriate support at time of death.
• Share in providing 24-hour, seven-day a week coverage to Home Health and Hospice patients/families through a rotating schedule and call schedule.
• Assist nursing staff in other departments as requested and qualified when need arises.

SECONDARY FUNCTIONS

• Schedule home visits as assigned.
• Alternate on call hours with other RNs.
• Participate in in-service programs, D/C planning, case conferences, Interdisciplinary Team meetings and staff meetings.
• Complete all Care Plan documentation, summaries and care planning on primary patients.
• Coordinate treatment with paramedical personnel.
