Job Purpose

In alignment with company mission, vision and values, the Care Manager partners with Operations to coordinate and manage service delivery to clients, optimizing their functioning, independence, and overall well-being.

Responsibilities
  • Uses care management skills to independently assess and reassess client status, including physical and mental health, social resources, environmental safety, economic resources, and functional ability.
  • Drafts, implements and revises the client plan of care to ensure appropriateness, effectiveness, efficiency, and client satisfaction.
  • Collaborates with clients, families, and support teams to obtain information, provide feedback, and facilitate healthcare coordination.
  • Maintains required documentation and utilizes effective communication to facilitate continuity of care.
  • Provides supportive communication and conflict resolution with clients and family members; serves as an advocate for client well‑being.
  • Makes referrals to appropriate community agencies (crisis intervention, legal advocacy, counseling) for medical, mental, physical, financial, legal, and social needs.
  • Communicates elevated care issues with appropriate internal professionals, including Area Director and Executive Team.
  • Extends client length of service through high‑quality care management and consistently exceeding client expectations.
  • Works closely with teammates who interview, match, train, and supervise in‑home teammates to ensure client needs are met when caregiving services are required.
  • Performs all responsibilities in accordance with professional codes of ethics, Arosa policies and procedures, and applicable state and federal regulations.
  • Achieves productivity target of at least 15 billable hours per week (some offices may have higher target).
  • Serves as Care Manager on call, managing client needs during evenings and weekends per on‑call rotation.
  • Documents and updates activity in multiple systems to ensure compliance with company protocols.
  • Participates in weekly peer supervision and monthly Care Management meetings (providing education as needed).
  • Collaborates with community organizations to promote elder care awareness and Arosa services through presentations, meetings, and outreach.
  • Participates in Arosa and office committees, programs, initiatives, and meetings as needed/requested.
  • Other duties as assigned.
Qualifications
  • Bachelor’s degree in Social Work, Nursing, Gerontology, Psychology, Health and Human Services, or related field (Master’s preferred).
  • Licensed or eligible for licensure in the professional field highly desired.
  • Minimum 2 years of care management experience; experience with aging population preferred.
  • Care Management Certification (CCM or CMC) desired.
  • Strong interpersonal communication skills and ability to work effectively with client support teams.
  • Team‑oriented work style.
  • Excellent written and verbal communication skills.
  • PC literacy.
  • Valid driver’s license and proof of current auto insurance.
Physical Demands
  • Travel by automobile.
  • Available beyond standard office hours, including evenings and weekends.
  • Adequate strength, endurance, and flexibility to perform job responsibilities; may require extended standing or sitting.
  • Use of standard office equipment and technology.
