Job Summary:The MDS Coordinator is responsible for coordinating and overseeing the completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations, including guidelines established by the Centers for Medicare & Medicaid Services (CMS). This role ensures accurate clinical documentation, appropriate reimbursement under the Patient-Driven Payment Model (PDPM), and compliance with all regulatory requirements within the Skilled Nursing Facility (SNF).
The MDS Coordinator collaborates with the interdisciplinary team to assess resident care needs, develop comprehensive care plans, optimize quality outcomes, and support survey readiness. This position plays a critical role in case mix management, quality reporting, and maintaining financial integrity through accurate coding and documentation.

Key Responsibilities:Coordinate and complete MDS assessments (OBRA, Medicare PPS, and other required assessments)
Ensure compliance with CMS regulations and state guidelines
Conduct chart audits to ensure documentation supports coding
Lead interdisciplinary care plan meetings
Monitor Case Mix Index (CMI) and PDPM reimbursement accuracy
Track and submit required Quality Measures (QMs)
Provide education to nursing staff on documentation and regulatory updates
Support facility survey readiness and respond to audit requests
Completes all other duties as assigned.

Qualifications:Required:
Active RN license instate of New Mexico orlicensedin any US state and be eligible for New Mexico licensure by reciprocity.(in good standing)
Minimum 2-3 years of SNF clinical experience
Strong knowledge of MDS 3.0 and PDPM
Excellent organizational and communication skills
Experience with electronic medical records (EMR) systems
Preferred:
RAC-CT certification
