• Continuity of Care:Develop long-term relationships with residents and truly make a difference in their daily lives.

• Provide direct nursing care to residents, including assessments, medication administration, wound care, and documentation.
• Monitor residents’ conditions and report changes to the healthcare team promptly.
• Implement care plans and contribute to their development based on resident needs and goals.
• Collaborate with RNs, CNAs, physicians, and family members to ensure quality care.
• Support residents with chronic illnesses, mobility limitations, cognitive decline, and end-of-life care.
• Adhere to all policies, procedures, and regulatory guidelines.

• Active Licensed Practical Nurse (LPN) licensein the state of New Mexico.
• Prior experience inlong-term care, geriatrics, or skilled nursingpreferred.
• Compassionate, patient-centered, and a strong communicator.
• Ability to work independently and within a team environment.
