- Provide direct nursing care to residents, including assessments, medication administration, wound care, and documentation.
- Monitor residents’ conditions and report changes to the healthcare team promptly.
- Implement care plans and contribute to their development based on resident needs and goals.
- Collaborate with RNs, CNAs, physicians, and family members to ensure quality care.
- Support residents with chronic illnesses, mobility limitations, cognitive decline, and end-of-life care.
- Adhere to all policies, procedures, and regulatory guidelines.
- Active Licensed Practical Nurse (LPN) license in the state of New Mexico.
- Prior experience in long-term care, geriatrics, or skilled nursing preferred.
- Compassionate, patient-centered, and a strong communicator.
- Ability to work independently and within a team environment.
