NM RN MAT Program

Under general supervision, responsible for delivering care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation/outcomes, in accordance with the established philosophy.

• Starting Salary: $55,000

• Weekly Travel is Required to Portales, Tucumcari and Ft. Sumner.
• This position will be working with current MAT provider daily, Monday - Friday.

• Screens, conducts assessments including vital signs & observed data, conducts intervention (Patient Education), and conducts evaluations.
• Communicates with other professional disciplines to ensure effective patient care delivery and achievement of desired patient outcomes.
• Interacts with patients and significant others, while maintaining the standard of professional nursing in accordance with state law and HIPAA mandates.
• Plans, provides support and nursing care to people who have a range of mental health issues in an ethical and safe milieu. Interviews and assesses the patient to learn about their history, symptoms, ailments, and daily habits. Identifies the mental health needs of the patients.
• Collaborates as part of treatment teams toward the development of individualized patient plans, aimed to maximize care and help patients to live productive lives; while assisting them towards independence.
• Charts, records, and documents. Records all care information concisely, accurately, and completely, in a timely manner, in the appropriate format and to appropriate forms.
• Prioritizes and researches patient information. Monitors for adverse side effects of medications.
• Follows through with the Team (prescribers, pharmacy, therapists, social workers, labs or diagnostics). Reports changes in the patient.
• Follows up with the patient/POA or legally designated family member (phone message response etc.).
• Travels within the community or various satellite facilities/operations; locations consist of Clovis, Tucumcari, and Portales.
• Reviews charts and ensures the chart is complete with pertinent data such as any recent discharge summaries. Notifies appropriate parties for missing information. Follows up to confirm the missing info is added to the chart.
• Follows-up including active response to phone messages with documentation/informational note.
• Address treatment plans & assignments. Monitor treatment regimens.
• Main HIPAA and client confidentiality at all times.
• Regular and consistent attendance for the assigned work schedule is essential.
