Communicate regularly with residents and their families about care plans, PT, OT, and treatment protocols. Attend and participate in morning meetings to facilitate communication. Answer resident questions, monitor and adjust resident status, and prioritize daily work. Complete necessary documentation and perform utilization review activities. Coordinate care, admissions, discharges, and referrals. Provide leadership and ensure team collaboration.

• Communicate with residents, families, and multidisciplinary team.
• Attend morning meetings and stand‑ups.
• Monitor resident status and adjust care plans.
• Prioritize daily workload and manage admissions/discharges.
• Complete accurate charting and documentation.
• Perform utilization review and coordinate care with payers.
• Facilitate referrals within and outside the facility.

• Bachelor’s Degree in Nursing or Social Work; RN license preferred.
• Registered Nurse license preferred; LVN/LPN acceptable.
• Two (2) years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid, and related benefit programs.
• Strong communication, time management, and independent judgment.
• Intermediate computer skills and ability to complete paperwork timely.
• Maintaining licensure and continuing education required.

Standing, walking, reaching with hands, pushing/pulling, lifting to 50 lbs, climbing, balancing, kneeling, crouching, and crawling occasionally. Ability to hear, taste, smell, and vision adjustments.

Low to moderate noise, reasonable accommodations for disabilities, and flexible scheduling. Noise level is usually low to moderate.
