• Complete and document comprehensive psychosocial assessments with patients in the electronic medical record.
• Identify care planning goals and develop interventions to promote best outcomes.
• Participate in Interdisciplinary Team meetings; provide insight into the psychosocial and emotional needs of patients.
• Provide education related to funeral planning and advanced directives as needed.
• Conduct visits with the patient and caregiver in their residential setting.
• Assist with bereavement support and interventions for caregivers following the death of a patient.
• Participate in an on‑hours rotation to make after‑hours visits when needed for a patient’s death, funeral, admission, etc.

• Tennessee and Missouri: MSW preferred, BSW required.
• Virginia: MSW preferred, BSW required.
• South Carolina:
  Social Work Licensure Required; LMSW required.
• Georgia:
  Social Work Licensure required; LMSW preferred, LBSW required.
• One (1) year of social work experience in a health care setting.
• Excellent communication and interpersonal skills.
