- Assume responsibility for the effective coordination of patient/caregiver care
- Conduct nursing assessments and develop, implement, and evaluate plans of care, including therapeutic, preventive, and palliative nursing actions.
- Make visits to provide appropriate interventions, following the established plan of care, evaluating progress, and revising plans as necessary.
- Provide emergency triage by telephone and home visits.
- Facilitate patient and caregiver independence to the extent possible.
- Initiate and maintain liaison relationships with health‑care providers and the interdisciplinary team, and communicate pertinent information reliably.
- Identify and refer psychosocial needs to VIA Health Partnersâs Medical Social Worker and provide education and instruction to patients and caregivers.
- Demonstrate nursing care procedures and utilize teaching materials appropriately.
- Ensure hospice care policies, protocols, standing orders, and guidelines are followed.
- Indirect patient and caregiver care responsibilities
- Maintain professional relations with external agencies; keep supplies safe and work area clean.
- Apply infection control principles and universal precautions.
- Keep clinical records current and submit all required documentation promptly.
- Maintain current medication profiles and document teaching.
- Record time and mileage accurately.
- Responsible for related duties as required and/or assigned

- Maintain professional reputation, pursue continuous development, and stay abreast of current trends.
- Maintain current licensure/certification (RN, CPR, CEUs).
- Assume responsibility for all material reviewed at meetings, whether in attendance or not.
- May perform other duties as required.
Minimum Qualifications
- Diploma or Associate’s Degree in nursing;
Bachelor’s Degree preferred. - Current, active, unrestricted compact (multistate) RN license.
- Current CPR required; CHPN preferred.
- Minimum 2 years of related experience in direct care of patients with life‑limiting illnesses (hospice, palliative, family medicine, internal medicine, geriatrics, oncology, home health, medical‑surgical). Pediatric experience may be required for specific roles.
- Computer proficiency.
