A Patient Care Coordinator is an entry‑level office role that is responsible for maintaining pleasant and consistent daily operations of the clinic. Our Patient Care Coordinators have excellent customer service skills and learn new things – a lot! The Patient Care Coordinator multitasks in multiple computer programs each day.
A day in the life- Greets everyone who enters the clinic in a friendly and welcoming manner.
- Schedules new referrals received by fax or by telephone from patients and physician offices.
- Verifies insurance coverage for patients.
- Collects patient payments.
- Maintains an orderly and organized front office workspace.
- Other duties as assigned.

The Patient Care Coordinator - I (PCC‑I) supports clinic growth through excellence in the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures and to support the mission, vision, and values of Upstream Rehabilitation.
Responsibilities- Core responsibilities
- Collect all money due at the time of service
- Convert referrals into evaluations
- Schedule patient visits
- Provide excellent customer service
- Create an inviting clinic atmosphere
- Make all welcome calls
- Monitor and influence arrival rate through a great customer experience
- Practice Management: manage schedule efficiently, document routing, personal overtime, non‑clinical documentation, deposits, caseload, discharge candidate, progress notes, and insurance reporting; monitor clinic inventory
- Training: attend required training with Territory Field Trainers (TFT) for updates; complete quarterly compliance training
- High School Diploma or equivalent
- Strong communication skills; able to relate well to Business Office and Field leadership
- Ability to multitask, attention to detail, ability to meet deadlines, operate with minimal supervision
- Team player with efficient time management and presentation skills
- Indoor environment; protections from weather, with possible temperature variations
- Full-time schedule, typical hours Monday–Friday, with a minimum of 40 hours per week; some evenings/weekends for events or extended hours may be required
- Sedentary work; ability to sit for long periods
- Constantly sits, with ability to stand when needed; communicates clearly with associates
- Regular use of computer and office equipment
- Occasionally lifts 0–20 lbs.; ability to lift up to 20 lbs as needed
- Repetitive typing and near-visual work
