Purpose:

_
** As a Clinical Support Specialist, you'll play a vital role in supporting Nursing Services by ensuring accurate documentation, smooth resident transitions, and efficient day-to-day neighborhood operations. This position is key to keeping our clinical teams organized and our residents well-supported - all while living out the Mission, Vision, and Values of Trinity Health Senior Communities

What you will do:

* + Support nursing staff with accurate medical and administrative documentation.

+ Maintain daily census reports and current resident rosters.

+ Coordinate resident admissions, transfers, and discharges.

+ Schedule appointments and assist with transportation arrangements for diagnostic and therapeutic services.

+ Ensure medical records are complete, updated, and properly routed to Medical Records.

+ Enter documentation accurately into Care Tracker and other electronic systems.

+ Communicate diet changes and important updates to appropriate departments.

+ Answer phones, route calls, manage faxes, and provide general administrative support to the neighborhood.

+ Serve as a key communication link between nursing staff, residents, families, and other departments.

+ Participate in ongoing education and required trainings.

Minimum Qualifications:

* + High school diploma or equivalent required.

+ Knowledge of medical terminology preferred.

+ Experience in long-term care or working with older adults preferred.

+ Proficiency with Microsoft Office (Word, Excel, PowerPoint) and electronic systems.

+ Strong organizational skills with attention to detail.

+ Excellent verbal and written communication skills. Professional, customer-service focused approach when working with residents, families, and staff.

+ Ability to multitask and thrive in a fast-paced, team-oriented environment.

+ Self-motivated with strong accountability and sound decision-making skills.
