Description
The Clinical Integration Specialist assists and supports the Director in quality and safety strategies, plans, and regulatory requirements that improve outcomes and reduce harm to patients. The specialist works closely with ministry colleagues, providers, and leaders in implementing, monitoring, and evaluating quality and safety programs, structures and requirements that culminate in high reliability.

Shift DescriptionThe Clinical Integration Specialist assists in achieving excellence in quality and safety work related to patient outcomes (process and outcome measures), performance improvement strategies and methodologies, culture of safety (Together Safe) initiatives, and patient safety organization (PSO) collaboration with risk management to ensure high quality and safe care. The specialist reports directly to the Regional Director of Quality, Access and Growth for the Medical Group.
Responsibilities- Knows, understands, incorporates, and demonstrates the Mercy One (Trinity Health) Mission, Vision, Core Values, Actions and Promise in behaviors, practices, and decisions.
- Assists the manager in oversight of quality, patient safety (including the culture of safety (Together Safe)) and infection prevention across the ministry.
- Supports the work of quality and safety related to the quality assurance and performance improvement plan, local/region/system strategies, regulatory and accrediting body requirements (CMS, IDIA, TJC, etc.), in conjunction with hospital leaders, colleagues and providers.
- Identifies problem areas in need of focused attention and supports the manager to implement actions to drive improved outcomes.
- Assists the manager in achieving outcomes aligned to CMS, pay-for-performance programs (including those aligned to commercial carriers) and TJC in areas of quality measures and regulatory requirements.
- Promotes continuous improvement toward achieving excellent outcomes and recognition with external rating organizations (e.g., CMS, Leapfrog, Health grades, Blue Distinction).
- Collaborates with Medical Group executive leadership, providers, and leaders to meet/exceed standards of excellence and achieve national designations, certifications, and awards.
- Collaborates with colleagues in accreditation, risk management, patient experience, clinical practice and performance excellence to provide needed support for improvement.
- Serves as a resource for Clinic Operations and Clinical Leaders in the development, implementation, and evaluation of patient care programs and clinic practice standards, including policies, guidelines, procedures and professional practice models.
- Facilitates and supports the local market Medical Practice Committees.
- Assists with implementation of quality excellence systems that integrate LEAN and other methodologies to create data-driven quality improvement programming.
- Provides appropriate support and/or consultative services (project design, data requirements, documentation tools) to quality improvement teams.
- Works closely with colleagues to improve the organization s culture of safety, including Just Culture and Together Safe. Supports opportunities for improvement on cause analyses, HFMEA, and PSO initiatives.
- Assists the manager in ensuring the accuracy of data in regulatory and accrediting quality databases (CMS HARP, TJC), registries, and dashboards/scorecards for internal and external customers.
- Promotes an environment of shared leadership, decision-making and accountability.
- Maintains knowledge of applicable Federal, State, and local laws and Trinity Health s Organizational Integrity Program, Standards of Conduct, and other policies to ensure ethical, honest practice. Supports organizational evidence-based practice, innovation and research. Assists the manager in the clinical contracting process, consistent with legal and regulatory requirements.
