h/care is pleased to announce the availability of a full-time position for an Experienced Hospice Registered Nurse Case Manager (RNCM) serving the regions of Aurora, Hudson, and Stow. Duties and Responsibilities- Conduct comprehensive patient assessments to identify their physical, emotional, and spiritual needs.
- Create personalized care plans in coordination with an interdisciplinary team to ensure optimal patient comfort and wellness.
- Provide direct nursing care to patients as outlined in the care plan.
- Maintain ongoing communication with patients, their families, and the hospice care team to facilitate open dialogue and decision-making.
- Administer medications and treatments as prescribed and monitor patient response.
- Act as a compassionate advocate for patients and their families to support them through the hospice process.
- Educate patients and families on the disease process, hospice care principles, coping skills, and end-of-life issues.
- Ensure documentation is complete, accurate, and complies with legal, regulatory, and clinical procedures.
- Participate in regular interdisciplinary team meetings to review and adjust care plans as patient needs evolve.
- Maintain the highest levels of confidentiality and professionalism during interactions with all parties.
- Navigate complex ethical and emotional situations with sensitivity and professionalism.
- Uphold the mission, vision, and values of h/care hospital, contributing positively to team morale and patient satisfaction.
- Valid RN license in the state of employment with no history of disciplinary actions.
- Minimum of two years of hospice nursing or palliative care experience.
- Demonstrated ability to cover and manage care responsibilities in Aurora, Hudson, and Stow regions.
