Under direction of the Nurse Manager and working as part of the Psychiatric Care Team, the Nurse is responsible for conducting psychiatric assessment; assessing physical health needs; providing management and administration of medication in conjunction with a psychiatrist; delivering a range of treatment, rehabilitation, and support services; and sharing shift‑management responsibility with other staff members. The nurse will provide nursing‑care visits, which include long‑acting injection treatments.

Through provision of services, the Nurse promotes healthy lifestyles and independence.

This position is Monday‑Friday, from 8:00 am‑5:00 pm.

Job Qualifications
  • An unencumbered Registered Nurse license in the state of Ohio
  • One (1) year of experience working with adults with severe mental illness
  • CPR certification and non‑violent crisis intervention training (within 60 days of hire)

Job Duties & Responsibilities
  • Provides registered‑nurse care appointments for patients
  • Assesses patients’ physical status, mental status, and psychiatric symptoms, including psychotic symptoms and safety factors
  • Refers patients for case worker or counseling when appropriate
  • Assesses medication adherence, manages medication system, administers and documents medication treatment
  • Screens and monitors patients for medical problems and side effects
  • Collaborates with shared appointment providers, especially when a patient is not doing well or requires medication change or dose adjustment
  • Provides bridge appointments: reviews hospital discharge orders, shares appointments with providers, enters new orders into Epic, and records hospital information such as reason for hospitalization and other relevant items
  • Assists patients with scheduling primary‑care provider visits
  • If a provider is not available for a shared appointment, assesses patient stability on current medications and sends refill requests to the provider
  • Coordinates, implements, and administers intramuscular injections
  • Accesses OARRS reports for patients on controlled medications
  • Obtains urine drug screens for patients on controlled medications when ordered by a provider
  • Communicates and coordinates services with other medical providers
  • Engages in health promotion, prevention, and education activities (assesses risky behaviors and facilitates behavior change)
  • Meets with patients individually to provide comprehensive medication education, monitor side effects and medication effectiveness, assist patients in developing systems to assure medication compliance, and involve family or significant others as needed
  • Educates other team members to monitor psychiatric symptoms and medication side effects
  • When patients consent, develops strategies to maximize medication adherence (e.g., behavioral tailoring, individualized cues, and reminders)
  • Works with the program psychiatrist to initiate and stabilize patients on medications
  • Assists patients in obtaining medications through assistance programs as needed
  • Participates in clinical team meetings to ensure appropriate and coordinated care
  • Provides consultation services regarding psychiatric and medical difficulties, as necessary
  • Monitors and records patient medication availability and usage, acting as liaison to pharmacies
  • Maintains organization and safety of medications by storing and disposing per policy
  • Performs shift management in coordination with other team members per established policies and procedures
  • Provides triage line coverage
  • Participates in daily staff organizational meetings and treatment‑planning review meetings

Physical demands:
This role operates in an outpatient, ambulatory clinic environment. Employees frequently sit, stand, walk, and use hand and arm movements, and may be exposed to infectious blood, body fluids, and airborne pathogens. Personal protective equipment is required, and travel or work outside normal hours may be necessary.
