Role SummaryThe Charge Nurse - LPN supervises the day‑to‑day nursing activities of the Certified Nursing Assistants (CNA) in accordance with company standards and federal, state, and local guidelines to ensure the highest degree of quality care is maintained at all times.
Essential Activities And Tasks Operations Management and Administration – 70%- Direct the day‑to‑day functions of nursing activities in accordance with current rules, regulations, and guidelines.
- Make work assignments to Nursing Assistants and coordinate nursing services for continuity of resident care.
- Administer medications and treatments per physician orders and nursing standards.
- Represent the nursing department during Interdisciplinary Care Plan meetings, answering questions related to the plan of care.
- Participate in committee meetings.
- Assist in identifying and correcting problem areas to improve care and services.
- Provide direct nursing care as needed.
- Consult with residents’ physicians for care planning, treatment, and rehabilitation.
- Notify physicians and families of any change in the resident’s condition.
- Authorize and monitor the use of restraints in accordance with facility policies.
- Admit, transfer, and discharge patients as necessary.
- Work with the Unit Manager to reduce hospital readmissions.
- Attend employee and departmental meetings and in‑service training.
- Ensure nursing procedures are followed by staff and reflect facility protocols.
- Participate in maintenance and implementation of the quality assurance program.
- Verify proper administration, storage, and control of medications and supplies.
- Ensure resident rooms and treatment areas are kept clean, sanitary, and safe.
- Supervise CNAs and foster a positive work environment.
- Interview, select, and orient new direct reports.
- Define and promote performance expectations.
- Evaluate job performance and provide developmental guidance.
- Document and address non‑compliance; work with HR when necessary.
- Participate in training program planning and execution.
- Ensure staffing plans are in place for absences or vacancies.
- Respond timely to staff requests and approvals.
- Current unencumbered license to practice as a Licensed Practical Nurse (LPN) in Ohio.
- One year of clinical experience in a hospital, long‑term care facility, or related setting (preferred).
- Supervisory experience in a long‑term care, hospital, or related healthcare setting (preferred).
- Proficiency with electronic medical records (EMR) (preferred).
- Must be able to read, write, speak, and understand English.
Special Requirements
- Sitting – up to 4 hours/day
- Standing – up to 8 hours/day
- Walking – up to 6 hours/day
- Lifting, transferring, pushing or pulling residents/patients or equipment/supplies – up to 50 pounds
- Driving – up to 6 hours/day
- Occasional overnight travel
- Work weekends, evenings, and holidays as needed
- 24/7 on‑call availability for emergencies
- Exposure to falls, burns from equipment, odors, cuts, various disease processes, infectious waste, and blood borne diseases (Risk Category I)
