The Nurse‑Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Maintains the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of The Metro Health System. Plans for, delivers, documents and evaluates nursing care of patients in an assigned area in terms of their individual needs, according to provider’s orders and in conformance with approved and recognized nursing standards of care, recognized nursing techniques, procedures and administrative policy.
Observes, assesses and reports on the condition of patients. Maintains accurate and timely documentation of care reflecting patient’s condition, treatment, and response to care. Upholds the mission, vision, values, and customer service standards of The Metro Health System.
- Required:
Graduate of an accredited school of professional nursing (or a program seeking accreditation). Current registered nurse licensure in the State of Ohio. - Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire.
- Basic knowledge of nursing theory and practice, including biological, physical, psychosocial, and medical sciences and their applications.
- Excellent written and verbal communication skills.
- Basic computer skills.
- Reliable transportation to travel to patients’ homes and Metro Health locations based on operational needs.
- Ability to work a flexible schedule including some evenings and weekends.
- Ability to travel to Denver, CO for all face‑to‑face education sessions.
- Ability to use computer.
Preferred:
Home visiting experience preferred. Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred. Bilingual (English and Spanish).
Physical Demands:
May need to move around intermittently during the day, including sitting, standing, stooping, bending, and ambulating. May need to remain still for extended periods, including sitting and standing. Ability to communicate in face‑to‑face, phone, email, and other communications. Ability to read job related documents.
