Your Precious Dreams is an intensive outpatient program located in Cleveland, Ohio, dedicated to supporting individuals facing addiction, mental health challenges, and behavioral disorders. We believe that recovery is a journey, and our flexible programs are customized to meet individual needs, whether clients are beginning their treatment or pursuing long-term support. Offering sessions at various times throughout the day—morning, afternoon, and evening—we provide a structure that enables participants to continue their daily routines while receiving the necessary care and guidance.

This is a full-time, on-site role for a Case Manager based in Cleveland, OH. The Case Manager will be responsible for providing comprehensive support to clients, including assessing their needs, creating personalized care plans, coordinating services, and connecting clients with resources to support their recovery journey. Additionally, the Case Manager will monitor progress, maintain accurate documentation, and collaborate with healthcare providers to ensure clients receive necessary treatment and assistance.

• Case management, client assessment, and care coordination skills
• Strong interpersonal and communication skills to effectively support clients and collaborate with team members
• Organizational and documentation skills, including the ability to maintain concise, accurate client records
• Understanding of addiction recovery, mental health care, and behavioral health treatment options
• Ability to work independently in an on-site environment and maintain confidentiality
• Previous experience in social work, psychology, counseling, or a related field is highly beneficial
• A bachelors degree in social work, psychology, or a related discipline; relevant certifications are a plus
